Job Description:

1. Must have knowledge of special education and/or behavioral supports for student in PreK through grade 12.
2. Act as liaison between assigned schools, agencies, parents and special services staff.
3. Assist staff in member districts with evaluation, placement, and services for students with social/emotional or mental health needs.
4. Assist school special education staff with secondary transition planning.
5. Collaborate and consult with special educators regarding student behavioral performance, progress, and limitations.
6. Identify behavioral goals on the Individual Education Program (IEP) and help develop strategic interventions.
7. Agency liaison with all, in and out-of-state social service agencies, hospitals, and treatment facilities and other agency teams as requested and approved by Director.
8. Agency liaison with mental health treatment providers and facilities.
9. Participate as a team member to provide help with referrals to appropriate community resources.
10. Liaison with responsible district and residential care facility.
11. Assist in the development of transition plans as children return to district services.
12. Collaborate with and update SVSS staff and Director regularly.
13. Maintain confidentiality among students, parents, professionals, and coworkers; and
14. Other duties as assigned by SVSS Director.
